PT-141 (bremelanotide) is a synthetic analog of alpha-melanocyte stimulating hormone (α-MSH) that acts on melanocortin receptors in the central nervous system. Unlike PDE5 inhibitors such as sildenafil, which work peripherally by relaxing blood vessels, PT-141 works centrally in the brain to activate sexual desire and arousal pathways.
FDA-approved in 2019 under the brand name Vyleesi for hypoactive sexual desire disorder (HSDD) in premenopausal women, PT-141 has since been used off-label by both men and women for libido enhancement and sexual dysfunction. Its central mechanism of action makes it effective even in cases where vascular interventions have failed.
The peptide typically requires subcutaneous injection 45 minutes to several hours before sexual activity. Because it acts on central pathways rather than vascular ones, PT-141 produces genuine desire and arousal rather than simply facilitating mechanical function.
